SagittalMeter is a professional-grade application designed to facilitate the measurement of sagittal balance parameters through direct placement of a smartphone on sagittal X-rays. This tool aims to support clinicians in accurately assessing spinal alignment without complex manual calculations.

Sagittal balance is a critical consideration in the management of spinal pathologies, especially during preoperative planning. An unbalanced sagittal profile can activate compensatory mechanisms that may compromise surgical outcomes. A thorough understanding of sagittal alignment is essential for determining appropriate interventions, including the selection of fusion or non-fusion devices.

The application allows for precise measurement of three fundamental angles crucial to sagittal alignment analysis across different clinical phases:
<ul>
<li>Pelvic Incidence (PI)</li>
<li>Pelvic Tilt (PT)</li>
<li>Sacral Slope (SS)</li>
</ul>

Additionally, lumbar lordosis—another key parameter—is measurable with SagittalMeter, including:
<ul>
<li>L1-S1 lordosis</li>
<li>L4-S1 lordosis</li>
</ul>

For comprehensive analysis, an advanced mode extends measurements to include additional parameters such as:
<ul>
<li>Occipito-C2 angle</li>
<li>Spino-sacral angle (SSA), defined as the angle between a line from the center of C7/T1 to S1 plateau and the surface of S1</li>
<li>C7/T1 slope</li>
<li>C7-C2 angle</li>
<li>Spino-cranial angle (CIA), measured between a line from C2 Dens to C7/T1 center and the surface of C7/T1</li>
</ul>

Understanding these measurements assists clinicians in evaluating sagittal imbalance, which is vital during preoperative planning. Restoring a patient-specific sagittal profile aims to achieve an economical posture, thereby optimizing surgical outcomes. While reference normal values are available, their estimation can be complex; SagittalMeter simplifies this process by providing quick assessments along with average normative data derived from asymptomatic healthy individuals.

In practical terms, assessing the need for correction before lumbar fusion involving L4–S1 is imperative. Discrepancies between PT or lumbar lordosis and PI values suggest the necessity for correction. The FBI technique—evaluating C7 translation angle, femur obliquity, and PT compensation angle—serves as a useful approximation. SagittalMeter facilitates calculation of the FBI index, offering an evidence-based estimate of the correction required to restore optimal sagittal balance.
